Job Summary:

This position is responsible for internal auditing and maintenance of the individual’s record through the Precision Care electronic system. In conjunction with the Quality Improvement and Program Coordinator for SPDD is responsible for:

Ensuring that the Life Plan has been received by the agency and uploaded into Precision Care. Verifies that the Life Plan accurately describes all the services being provided to an individual.
Tracking due dates for the Life Plan and Staff Action Plans.
Ensuring completion of the Monthly Review Notes.
Tracking the Staff Action Plan distribution to Care Coordinators.
Ensuring and tracking annual Level of Care.
This person will ensure the program is in compliance with agency policies and regulations from NYS OPWDD and OMIG. They will also manage the accuracy of all data utilized for billing and funding reports.

Duties and Responsibilities:

Works with the Director of SPDD, all program staff and the QI Department to ensure compliance with all applicable regulatory and agency requirements.
Utilizes the Precision Care electronic system to manage and monitor all SPDD data and information and ensure it is complete, accurate and in compliance.
Utilizes reports in Precision Care to monitor due dates for all compliance documentation.
Manages the accuracy of all data utilized for billing and funding reports.
Participates in collaborative problem solving with program staff to address immediate issues and strengthen program operations.
Becomes an expert in all aspects of the Precision Care system.
Conducts on-site record reviews as needed.
Interacts with Care Coordinators, day programs, nursing, clinical and medical, and all other habilitation providers to obtain necessary documentation.
Trains and supports staff in understanding their roles and responsibilities related to documentation and entering data into Precision Care.
Performs related duties and completes special projects as requested.

Qualifications:

Proficiency in data collection, assessment, collaborative problem solving, facilitation, written communication, and strategic planning.

Bachelor’s degree required.

Candidate must be proficient in the use of a computer and appropriate software including Word and Excel.

Experience with OPWDD system preferred.

(35hrs a week)

Job Type: Full-time

Salary: $53,000.00 - $55,000.00 per year
