Job Description

Abbott House is seeking a passionate Social Worker to join the Group Homes team. In this role you will be a liaison between the youth in the programs and the relevant stakeholders including those who support them, their educational agencies, service providers, their county and state agencies and other appropriate individuals associated with their care and treatment.

Key Responsibilities

Write treatment plans and the individual crisis management plans
Complete all FASPs and Permanency Hearing reports
Facilitate home visits, family visits, sibling visits for the youth as well as communication between the youth in the program and family members.
Assist family members in understanding the treatment for their child and how to apply these skills and interventions to other settings.
Provide comprehensive service coordination from initial assessment to obtaining appropriate and adequate services for each resident.
Arrange appropriate transition services for a youth nearing discharge from the program. Each residence will have its own Social Worker/Case Planner

About You

You are the ideal candidate if you have a passion to empower your community and help others. Being a self-starter, driven, flexible and responsive are critical attributes to being successful in this role.

You have at least a Bachelor’s degree with experience in Child Welfare. Master’s degree is preferred.
You have a minimum of 2 years of experience in child welfare roles
You have professional maturity and demonstrate the ability to handle sensitive and confidential situations
You have outstanding interpersonal and communication skills
You are technology savvy with excellent command of a variety of computer software applications in word processing, spreadsheets, database, and presentation software (MSWord, Excel, Access, PowerPoint)

Job Type: Full-time

Salary: $46,222.00 - $47,222.00 per year
