The Waiver Supervisor oversees Staff CANS Assessment and Health Home Appropriateness Screening Assessments on all new referrals, determining qualifications for Health Home admission, assures all necessary client consents for service are demonstrated, provides guidance on outreach in designated venues to generate program referrals as well as develop a network of referring entities and provides weekly supervision and monitor Outcomes for Benchmark on a monthly basis.

Education & Experience
Masters’ Degree in one of the following Fields: Audiology, Child and Family Studies, Communication Disorders, Community Mental Health, Counseling, Education, Nursing, Occupational Therapy, Physical Therapy, Psychology, Recreation, Recreational Therapy, Rehabilitation, Social Work, Sociology, Speech and Language Pathology, Human Services, Human Development, Criminal Justice or other related degrees.

NYS License in related field preferred.

Two (2) years experience providing management of direct services to Severely Emotionally Disturbed (SED) children and adolescents or providing linkage to services for children with mental health diagnoses.
