Sign-On Bonus full-time position $3,500
The Caseworker is responsible for the Care Coordination of Case management cases in the Children's Waiver Program and the Targeted Case Management Program.

Duties & Responsibilities:

This includes assessment of new referrals, outreach to gain referrals and coordinating medical and mental health services with assigned Health Home.

Education & Experience

A bachelor related degree and 2 years of experience: providing direct services to Severely Emotionally Disturbed (SED) children and adolescents or providing linkage to services for children with one mental health diagnoses is required
A master degree and 1 year of experience: providing direct services to Severely Emotionally Disturbed (SED) children and adolescents or providing linkage to services for children with one mental health diagnoses.

Bilingual (Spanish) Preferred.
