Job Summary:
The Quality Improvement Coordinator will work with all of the SPDD residential and day habilitation staff to collect data and use evidence to develop measurable goals for program improvement using a Continuous Quality Improvement approach. This person will also ensure programs’ compliance w/agency policies and government regulations from NYS OPWDD and OMIG. This staff person will report to the Director and Assistant Director of QI and Program Development.
Duties and Responsibilities:
• Identify and participate in immediate response to safety related issues in programs.
• Develop knowledge of policies and regulations related to the agency and specific programs, and keep program staff updated.
• Assist and in some cases host Incident Review Committee.
• Conduct on-site case reviews.
• Gather and provide data for trend analysis.
• Assess data from a variety of methods and share findings with program staff.
• Develop data tracking tools using Excel, as needed.
• Facilitate the CQI process with program staff through four key annual activities.
• Participate in collaborative problem solving with program staff to address immediate issues and strengthen program operations.
• Complete all necessary internal and external reports related to program performance.
• Reports will include, but not be limited; Timeliness, and quality of progress notes; required casework contacts; Billing: Quality of Service Plans; Incident and Human Rights Review; Incident trends.
• Assess reports and audits received from government jurisdictions and manage follow up.
• Assist in agency incident investigations.
• Any other related duties, as required.
Proficiency in core competencies is important, including: data collection, assessment, collaborative problem solving, facilitation, written communication, and strategic planning.
• Bachelor's Degree Required with 3-5 years of experience
• Strong Excel Skills Required
• Experience with OPWDD system preferred
• Flexible with work hours
