About the Job:
In this position, you will be responsible for the development of psycho-social assessments and service plans for children and families to ensure child safety and reduce child risk in accordance with the Improved Outcomes for Children (IOC) initiative. You will be completing case management duties and one on one mentoring to a minimum caseload of 12 families including advocacy and assistance with entitlements.
• Provide one to one mentoring, coaching and goal setting, specifically around goals related to the promotion of economic mobility and increased family alignment for the head of household and her family, including children.
• Serve as a resource for the participant and family on issues including housing, budgeting, career and education, childcare, health care, and more.
• Conduct detailed assessments, monitor and document participants’ progress according to individual plans.
• Recruit participants for the program, including outreaching to community groups, presentations to local agencies, etc.
• Thorough and organized documentation of caseload efforts, including up to date and accurate documentation of electronic database system according to deadlines.
• Participate in the development and facilitation of participants’ workshops and community groups.
• Facilitates solutions to each family’s service delivery problems and identifies with client any potential barriers to care.
• Responsible for the overall management of the family’s Individualized Service Plan. Monitor and evaluate a client’s needs, including prevention, wellness, medical, mental health, care transitions, and social and community services where appropriate.
• Must meet documentation requirements in a timely and accurate manner by effectively utilizing designated Management Portal (CONNX, PROMIS, Safe Measures) and other Records as needed.
• Promotes wellness and prevention by linking families with resources and services based on their individual needs and preferences.
• Helps clients to obtain and maintain public benefits necessary to gain health care services, including Medicaid and cash assistance eligibility, Social Security, SNAP, housing, legal services, employment and training supports, and others.
• Effectively communicates and shares information with children and families and other caregivers with appropriate consideration for language, literacy and cultural preferences.
• Meet regularly and consistently with clients as per contract.
What We are looking for:
The ideal candidate would have the following education, skills and experience:
• Bachelor’s Degree
• Bilingual Skills (English and Spanish) preferred.
• Experience working with individuals who have experienced trauma and crises such as homelessness, domestic violence, poverty.
• Knowledge of case management, motivational interviewing and coaching practices.
• Excellent organization skills, written and oral communication skills and team work.
• Good computer skills with solid knowledge of Microsoft Office Suite.
• Ability to work flexible hours (evenings/Saturdays) as assigned.
• Able to conduct home/field visits and travel throughout the five boroughs as needed.
