Exciting Community-Based, Visiting Nurse RN Positions Available

Cardinal McCloskey Community Services has an exciting opportunity for a NYS Registered Nurse to provide care coordination to adults with developmental disabilities in various group home settings across Westchester County and the Bronx. As a Community-Based Visiting Nurse, you will fulfill your passion for helping others in the community while providing needed health guidance and teaching to the adults we serve as well as the dynamic staff that works tirelessly to meet their needs. Must have valid NYS Driver’s license and own transportation to travel across various work sites.

CMCS provides excellent nursing and clinical supervision and all employees receive free training in the trauma-informed, organizational change model known as the Sanctuary Model. Additionally, all eligible RN’s have the opportunity to receive free training in an interactive management training series known as Developmental Dimensions International (DDI) that supports professional development and growth as a future Nursing Supervisor.

Position Summary: The Full-Time Nurse Care Manager (35 hours weekly) is responsible for the well being of all the individuals under his/her care. Accordingly, the RN provides care management and medical consultation/triage services and medical education to the individuals, as well as education/training and surveillance to the staff for delegated nursing duties. Medical care coordination for all individuals in assigned houses is accomplished through ensuring that appropriate medical treatment and follow-up are provided, staff training, surveillance of AMAP staff for performance of delegated nursing tasks, on-call consultation and triage and participation in program planning / treatment team meetings.

Position Requirements:

BSN preferred
Registered Nurse with current New York State Licensure
Previous experience (preferably 3+ years) working with people with developmental disabilities and/or psychiatric disabilities preferred.
Previous experience providing health care management / community-based nursing
Previous experience working with a medically fragile population preferred
Must be willing to travel and make home visits across the entire NYC region (all 5 boroughs)
PPE will be provided including hand sanitizers
