Staff members at CMCS are our greatest asset! We are currently seeking qualified and eager Case Planners for our Family Foster Care program!
Responsible for the overall management of the provision of services to each child, foster boarding parent and biological family on his / her assigned caseload. This includes assessment of needs of each individual and the detailing and implementation of a treatment plan designed to effect the earliest possible discharge of each and every child to a permanent living situation.* The Senior Case Planner is also responsible for supportive coaching and guidance to the foster care program.
Schedule casework contacts in accordance with ACS requirements.*
Maintaining the relationship between and child and bio parent by sharing with each significant information about the other and facilitating frequent visiting in accordance with the standards set forth in Adm. 100, Utilization Review and ACS guidelines between bio parent and child.*
Maintaining regular contact with the foster parent, at least monthly, in order to facilitate the child’s adjustment to the foster home.* This includes sharing pertinent information about the child’s past, interpreting problem behaviors, and offering suggestions around more effective parenting techniques.*
Responsible for planning with child’s discharge resource including arranging services in the community for the child’s family members & discharge resource.*
Responsible for court appearances and testifying in court related to assigned cases.*
Responsible to set up, monitor and help youth maintain family connections through siblings visits and family visits with their biological family or discharge resource.*
Participate in Family Team Conferences, Service Plan Reviews, case consultations, and other functions as appropriate.*
Report any changes in child’s case planning or behavior to the Program Supervisor.*
Essential skills, experience, licensure, certification:
BA / BSW and 2 years Human Services experience required.
Bilingual in Spanish preferred
Work experience in Child Welfare &/or Office of Mental Health System preferred
NYS driver’s license preferred
Computer literacy required.
