Position Summary: The FT/R program is designed to address the safety and well-being of children in high-risk families when a caretaker or child’s substance use or mental health disorder places the child at risk of foster care placement or replacement. The model calls for treatment to be provided in three phases: Initial, Baseline and Stabilization. The service plan is guided by comprehensive assessments and a multi-disciplinary approach involving different interventions in order to meet the needs of the families that are being served. The expected case duration is an average of twelve months. The FT/R model utilizes a team approach to assess, diagnose and treat each family member’s needs.

Position Responsibilities:

Promotes a trauma informed, therapeutic and working relationship with families
Aides in the development of comprehensive assessments to identify the strengths and needs of children and/or parents with substance abuse and/or mental health needs while providing casework counseling that reinforces safety for children and reduces or eliminates risk factor
Upholds strong collaboration and communication with ACS and other stakeholders throughout the life of the case
Makes diligent efforts to meet the ACS minimum casework contact standards for a caseload of up to 10 families
Provides case work services that continually assess the level of family engagement in services, the extent of behavior change related to the facts and circumstances that led to the preventive referral, and the level of in-home functioning
Adheres to the Children’s Services Clinical Diagnostic Team Meeting and Family Team Conference model and incorporates the action plan into case practice
Educates family members on how their behaviors are impacting the family functioning and child welfare involvement
Refers the child/family to appropriate service providers including community based service organizations as well as city and state agencies, to promote safety and overall well being
Maintains contact with the service providers on a consistent basis for case planning and collaboration
Ensures that documentation is reflective of all case activity, best case practice and entered into the system of record contemporaneously with the event date

Essential skills, experience, licensure, certification:
BSW or Bachelor’s Degree in Human Services
Some knowledge of Child Welfare System
Counseling experience with children and families
Basic computer skills
Excellent verbal and written communication skills
Strong organization and time management skills
Flexibility to meet changing needs and demands
Fluency in Spanish and English language preferred
