| Enter a description of the Job | THIS POSITION HAS A $3,500 SIGN-ON BONUS The Care Manager will conduct mandated monthly client contacts, attend necessary appointments with clients, and develop the client’s Plan of Care by the required due dates to include the Department of Health (DOH) required 10 Care Plan elements. The Care Manager will complete the Child and Adolescent Needs and Strengths (CANS) certification, follow all Health Home policies and procedures, and perform other related tasks. An associate degree in a related human services field plus 3 years of experience providing direct services to Severely Emotionally Disturbed (SED) children and adolescents or providing linkage to services for children with one mental health diagnoses OR a bachelor’s plus 2 years of experience OR a master’s plus 1 year of experience is required. Bilingual Spanish/English is required. A valid non-restricted NYS driver license is required. |
